anti-TNFはpatient well-beingの改善が他のDMARDよりも良い
１個のDMARD（通常はMTX、最低15mg/weekを３ヶ月）でコントロールできない場合にanti-TNFを追加する。
anti-TNFはMTXを含むcombinationを試した後まで投与を延期すべきとは考えていない。
anti-TNFはsteroid sparingの目的で用いる。
anti-TNFはacute-flarreのmanagementの手段とは考えていない
肝、腎障害などでDMARDが使えない場合にanti-TNFは用いうると考えている
contraindicationと思われるもの：active or recent infection(TB,
 osteomyelitis,joint infection), previous malignancy(particularly lymphoma)
