RA患者においてステロイド誘発骨粗鬆症の治療がどのようになされているかしらべた。1999年1
月または2月に受診した623人のRA患者のうち236例（44％）にステロイドが投与されていた。ス
テロイド服用RA患者は、平均年齢60歳、女性80％、平均罹病期間は8.8年であった。68％はRF陽
性であった。23％がBMDを施行されていた。しかし、高用量服用者、長期服用者がBMDをやって
いる傾向はなかった。42％に何らかの骨粗鬆症治療薬が処方されていた。その内訳はアレンドレ
ネート（19％）、エチドロネート（2％）カルシトニン（10％）HRT（22％）であった。さらに
Vit　Dとカルシウムを治療薬として含めると61％が骨粗鬆症治療薬を飲んでいた。（注　どう
もVitDもカルシウムも病院で処方するくすりではないらしい？）BMDをやらない要因、薬を出さ
ない要因を統計解析したところ、男性、閉経前女性があてはまった。考察　リウマチ専門医は
DMARDSの副作用についてはよく観察しているのにステロイドの副作用を軽視している可能性が
ある。原因として、骨粗鬆症などのステロイドによる副作用は、皮疹や肺炎、蛋白尿なのどのよ
うに急性におこらず、長い年月をかけて進行することも考えられた。

古谷の感想　この論文をよんで昨年ACRが出したステロイド誘発骨粗鬆症を読み直しました。こ
れによれば、PSL換算5ｍｇ/日以上を長期に服用している患者では、BMDを行い異常低値なら、
ビスフォス（ボナロン、ベネット）の処方を薦めています。世界一症例数の多い当センターもそ
の方針に従うべきであると感じました。

最近の米国の自発的副作用報告によると、Etanercept 18例、Infliximab 8例のリンパ腫
発症報告があったが、10万人あたりの発症頻度は、 Etanercept 、Infliximab 、米国の
1992-98年の予測値の順に19.0,  6.6,  18.3で差がなかった。

これはRA治療ガイドラインに収載しました。
