  [Application for Confirmation of the Registered Items]

  If you are older than 16 years, you are required to apply for confirmation of the registered items within 30 days of the 5th birthday from the date of initial registration or previous confirmation. If not, you shall be subject to punishment. If not older than 16 years, the first confirmation shall be applied for within 30 days of your 16th birthday.

  Upon confirmation, a new certificate will be issued.


[Return of Certificate]

  You shall return your certificate, WHEN;

   1. You leave Japan without a re-entry permission.

       You are required to return it to an Immigration Inspector.
      When carrying a re-entry permission, you shall never return
       your certificate.

   2. You have acquired Japanese nationality or died.

       Your certificate shall be returned to the office of the city of
       your residence within 14 days.
